Registration Instructions


E-Z PASS

E-Z pass may be used for outdoor teams that are NOT being split.
When you come to registration, you must bring three copies of this roster form. Absolutely No cross outs. Go to the E-Z pass line.
If you are splitting your team, you may use your outdoor roster for verification purposes. You must follow the directions below as if you are registering a new team:

  1. Click on Registration Form Word Document (Below).
  2. Open With Microsoft Word.
  3. Fill in the names of your players.
  4. Select Files > Save as.
  5. Select Files > Print (Print 3 copies)


*If you have a MAC or cannot open a Word document
  1. Click on the Registration Form Adobe Document (Below)
  2. You will not be able to save form. You will have to type in the names and Print (3 copies)

New Team Registration

If you are registering a new team, you may follow the registration procedure above by typing in the names on the registration form from the web site or use the form that is in your packet.
You must bring age verification.


New Player Additions

Player additions must be put in the section on the roster form designated for player adds.
NEW PLAYERS (those who did not play in a fall sanctioned league): New players will only be issued the Indoor Soccer program tournament pass at a cost of $8.00 per/card no matter what association you are affiliated with. No new players will be registered unless the player-pass fee is paid at the time of registration, and the indoor soccer tournament card will be withheld until payment is received.
